How to build a tool. Tick the measures you want on the left. Set your follow-up timing on the right: each measure is auto-scheduled, and you can change when it is asked. Add story prompts if you want stories as well as numbers. Then download a Word document or save it to reuse. Each measure is tagged as a validated scale, a simple indicator, or a qualitative method.
